Does your business have a disaster recovery plan?

The recent spate of floods and tornados, mudslides and wildfires, hurricanes and dust storms has reminded me that I should probably put together a disaster recovery plan in case there is some kind of massive disruption in my company's ability to provide products and services to our customers. Have any of you done this before? Do you have any suggestions for what aspects I should cover?

In some industries, not having an adequate disaster recovery plan could be considered a crime. However, I prefer to think in terms of business continuity, not disaster recovery, with the focus being on how to keep the business functioning no matter what happens.
