It might be failing, but there is a known issue on some Crucial M4s called the 5200 hour bug. It sounds like your SSD might be old enough to have the problem. There is a firmware patch that might fix your problem. There is a nice guide on Storage Review that will walk you through the patch process. Hopefully that will take care of it. If not, be sure to check all your cables and connection before you start shopping for a replacement.
